The application automatically generates a form with basic fields, you just need to enable it. Please go to Dealer form on the left sidebar -> Active form tab -> Enable Form
Depending on your needs, you can completely customize the form easily.
In this tab you need to set up an email address to receive notifications each time the form is submitted. In addition, you also need to set up the content of the email to be suitable for the purpose (the application has automatically generated sample content that may not be suitable for you).
The application is integrated with 8 most popular email services: Mailchimp, Klaviyo, Campaign Monitor, GetResponse, Omnisend, Google Sheets, Active Campaign, HubSpot.
With this feature, you can collect customer emails for email marketing strategies.
Add form to page
At the Active Form tab, you get the short-code of the form, then you add it to the content of the page.

How to add the correct fields to create a valid location?
In the step of adding a new field, be sure to select the correct field type for the location listed in the select-box at the bottom.
This selection helps you to create standard fields for the form, and once the form is submitted, the location information will be pushed to the Location tab of the Submission details.
The final step of adding the submitted location to the Store locator page is to go to Dealer Submissions on the left sidebar-> Submission Details tab -> View details (Eye icon) -> Select Location tab -> Click the Add to location list button.
